The State of New Hampshire is making significant changes in the way your loved one's health care in provided and paid for. A new program called "Medicaid Care Management" will take the place of the state's traditional Medicaid program. If your loved one's medical care is paid for by New Hampshire's Medicaid program, these changes are important to your loved one's future care.
These changes begin taking effect on February 1, 2016. You have from now through January 4, 2016 to choose a new health plan for your loved one. If you have not chosen a plan by January 4, your loved one will be automatically enrolled in a program by the State of New Hampshire.
There are important choices and decisions regarding personal physicians, physician specialists, and drug coverage that you will have to make before January 4.
If you have not been to one of our workshops on these changes already, we encourage you to attend our last workshop on Thursday, November 19 at 5:30 pm in the Villages Main Lounge. During the workshop, we will do our best to help you make the choices that are in your loved one's best interests. If you've already received information about the upcoming changes, please bring that paperwork to the workshop.
