Our records show that an ACP Physician Engagement specialist conducted training in your office on implementation of the Million Hearts® campaign.
Unfortunately, we do not have a record of this training! We apologize for the inconvenience, but ask that you complete and return the attestation formto Shariff De Los Santos, ACP’s Cardiovascular Project Manager.
Million Hearts is a core element of ACP’s Cardiovascular protocol (DSRIP Project 3.b.i), which targets patients age 18 and above who have Cardiovascular disease of Hyperlipidemia. Implementation of Million Hearts must be documented by using ACP reporting code LSM01.
Thank you in advance. Your active participation in ACP’s DSRIP projects is essential to meeting our ever-increasing patient engagement and pay-for-performance metrics.
If we can be of service to your office, please let me know.
