HELP MAKE YOUR MILLION HEARTS®
TRAINING COUNT
Dear ACP Network Physician –

Our records show that an ACP Physician Engagement specialist conducted training in your office on implementation of the Million Hearts®  campaign.

Unfortunately, we do not have a record of this training! We apologize for the inconvenience, but ask that you complete and return the attestation form to Shariff De Los Santos, ACP’s Cardiovascular Project Manager.

Million Hearts is a core element of ACP’s Cardiovascular protocol (DSRIP Project 3.b.i), which targets patients age 18 and above who have Cardiovascular disease of Hyperlipidemia. Implementation of Million Hearts must be documented by using ACP reporting code LSM01.

Thank you in advance. Your active participation in ACP’s DSRIP projects is essential to meeting our ever-increasing patient engagement and pay-for-performance metrics.

PCMH Level 3 Certification: ACP Can Help!

Practices are required to achieve Level 3 Patient Centered Medical Home (PCMH) certification by December 31, 2017 as part of participation in DSRIP.

As a member of the ACP network, you are entitled to the services of a practice transformation specialist -- provided at no cost to you -- that will help your practice become PCMH Level 3 certified.

We recommend allowing six months to a year to complete the PCMH certification process. Click here to read more. Contact ACP's PCMH Team for more information.
