Care.com connects families with companions and caregivers to provide medical and non-medical assistance. Services include errands, transportation, bathing, exercise, and 24-hour coverage for Dementia and Alzheimer's patients.

 

CarePort Health collects and analyzes data from 100s of post-acute care organizations to identify providers that match a patient’s unique clinical needs and preferences. Higher rated providers, such as those with low readmission rates, are identified.

 

CareLinx is a professional caregiver marketplace, empowering families to easily find, hire, manage and pay licensed caregivers online. The platform has over 100k caregivers to match specific needs and budgets.

 

Caremerge creates a care coordination network that keeps care teams, families, and patients informed and connected through a HIPAA compliant, easy-to-use mobile and web-optimized interface.
